Please ensure you agree to all these statements:

  • You are completing this online registration for yourself, and you will not share any medication, that is later prescribed, with another person.
  • Typically, it will take around five minutes to complete, and your responses are confidential and securely stored. If you are ineligible for treatment, or later decide not to proceed with treatment, your data will be permanently erased after 6 months.
  • You will disclose any medical conditions, significant illnesses, or operations you have had, both recently and historically.
  • You will disclose any prescription medications you are currently taking.
  • You agree to use only one weight loss medication at a time.
  • You agree to our Terms and Conditions , and confirm that you have read our Privacy Policy.
  • The online application registration consists of a questionnaire. You must answer accurately and honestly to all sections. Withholding or providing false information can severely harm your health. By filling out this questionnaire, you confirm that your responses are truthful and accurate – and you acknowledge the potential risks to your health of any missing or misinformation that you provide.

Additionally,

  • If you are eligible for treatment, you will need to book and pay for a face-to-face appointment at Clarendon Health. Before a prescription for medication is supplied, a written consent form will also need to be completed, which you will need to sign. Our prescribing of weight loss medication meets the National Institute for Health and Care Excellence (NICE) guidelines. However, any medicines prescribed by a clinic (or a GP etc.) can have side effects. These will be discussed with you prior to starting treatment.
  • I understand that decisions about treatment will be taken jointly by a Clarendon Health Clinician, and myself (as the patient). However, the final decision as to whether to prescribe weight loss medication is that of the Clarendon Health Clinician at the face-to-face appointment.
